Telemedicine for Long Term Health and Post Acute Care

LTHCareline

Founded by nursing home professionals for nursing home professionals.

LTHCareline is an overnight, long-term care telehealth service that uses HIPAA-compliant technology and software to bring advanced clinical care to your patients on a smartphone.  Easy to hold and easy to transport, LTHCareline services are readily accessible to any of your nursing staff with a simple text message.

Your facility staff can reach us 24 hours a day to see and treat your patients via telehealth whenever it is convenient for them. In addition, our talented team will visit your facility regularly for in-person visits to build trust while ensuring continuity of care.

Get around-the-clock access to qualified and compassionate healthcare providers.

Our Services

Providing our patients with access to better care- at any time, from anywhere.

Because sudden illness or medical condition flare-ups don’t abide by business hours, our telehealth service ensures our providers are there when you need us. Our standardized protocols for common overnight emergencies such as falls, respiratory distress, and altered mental status provide a steady and consistent standard of care you can rely on to decrease your organization’s legal risk and avoid State allegations of negligence. Telehealth makes quality healthcare more accessible and improves patient outcomes and facility success.

In addition to getting your patients medical care when needed, knowing they have access to our providers via telemedicine offers peace of mind for patients and their loved ones.

Staffing Hours

Monthly, you receive reports on how many staffing hours you can claim for compliance with QIPP regulations for staffing and star rating requirements.

How Telehealth Works

We use a telehealth platform that allows us to visually see, hear, and interact with your patients. LTHCareline achieves this with a simple user experience – nurses can send HIPAA-compliant text messages via smartphone to request and receive a live clinician video call to assess the patient within minutes. After a virtual visit is scheduled, a medical assistant or other designated person at your facility will provide patient vitals and act as an intermediary between doctor and patient.

If desired, patient caregivers, family members, or other medical specialists may be invited to join the telehealth visit for an efficient team-based approach to individual patient care.

Around-the-Clock Access

Don’t just hire medical providers who can make periodic rounds at your skilled nursing facility or rehabilitation center – make sure the clinicians you hire are accessible to you and your patients when you need them.
